dc.description.abstractHypercalcemia of malignancy is a common finding thypically found in patients with advanced stage cancers. Hypercalcemia has been reported to occur 20-30% of patients who have a malignancy. Hypercalcemia of malignancy usually presents with markedly elevated calcium levels and therefore, usually severely symptomatic. Several major mechanisms are responsible for the development of hypercalcemia of malignancy including parathyroid hormone -related peptide-mediated humoral hypercalcemia, osteolytic metastases-related hypercalcemia, 1,25 vitamin D-mediated hypercalcemia, and parathyroid hormone mediated hypercalcemia in patients with parathyroid carcinoma and extra parathyroid cancers. Diagnosis should include the history, physical examination and measurements of the mediators of hypercalcemia. Management includes intravenous hydration, calcitonin, biphosphanates, denosumab, prednisone and cinacalcet. Patients with advanced underlying kidney disease and refractory severe hypercalcemia should be considered for hemodialysis. Oncology, hematology and palliative cancer care specialists should be involved for cancer targeted therapies.en_US
dc.description.abstractMaligniteye bağlı hiperkalsemi tipik olarak ileri evre kanserli hastalarda gorulur. Bu hastalarda hiperkalsemi gorulme oranı %20-30 oranında bildirilmiştir. Malignensi ile ilişkili hiperkalsemide genellikle kalsiyum duzeyi belirgin yuksektir ve bu nedenle belirgin semptomatiktir. Birden fazla mekanizma maligniteye bağlı hiperkalsemide rol oynamaktadır. Bunlar; paratiroid hormon ilişkili proteine bağlı humoral hiperkalsemi, osteolitik metastaza bağlı hiperkalsemi, 1,25 Vitamin D ile ilişkili hiperkalsemi ve paratiroid karsinomunda ve ekstra paratiroid kanserlerde gorulen PTH ile ilişkili hiperkalsemilerdir. Tanıda hastanın hikayesi, fizik muayene ve serumda hiperkalsemi ile ilgili medyatorlerin olculmesi onemlidir. Tedavide intravenoz hidrasyon, kalsitonin, bisfosfonatlar, denosumab, prednizon ve cinacalcet kullanılabilir. Renal ciddi hastalığı olan ya da refrakter hiperkalsemisi bulunan hastalar icin hemodiyaliz duşunulmelidir. Ayrıca hastaların malignitelerinin ilgili hematoloji ve onkoloji bolumleri tarafından medikal veya palyatif olarak tedavi edilmesi gerekir.en_US
