YÖNTEM:Cretan plus protokolü uygulanan 16 keratokonus hastasının 20 gözü çalışmaya dahil edildi. Hastaların preoperatif, postoperatif 1., 6. ve 12. ay refraktif, topografik, ve optik kalite verileri Sirius topografi (CSO, Italy) cihazı ile değerlendirildi. Tüm hastaların aberasyon profili (yüksek sıralı aberasyonlar (HOA), vertikal koma, horizontal koma, vertikal trefoil, oblik trefoil ve sferik aberasyon) ile vertikal ve horizontal modülasyon transfer fonksiyonu (MTF) ve nokta dağılım fonksiyonu (PSF) şeklinde optik kalite parametreleri karşılaştırıldı. BULGULAR:Amsler-Krumeich keratokonus sınıflamasına göre, 6 (%31.2) göz evre I, 14 (%68.7) göz ise evre II keratokonus idi. Ortalama Kmax değeri ameliyat öncesi 51.16 ± 1.90 D’dan ameliyat sonrası 1. yıl 47.06 ± 1.68 D’e düştü (P<0.001). Ortalama düzeltilmemiş ve düzeltilmiş uzak görme keskinliği, korneal astigmatizma ve manifest sferik ekivalanda 1. yılda anlamlı azalma gözlendi (P<0.001). Ayrıca, Cretan plus protokolü uygulaması sonrası 1. Yılda HOA, vertikal koma ve sferik aberasyon değerlerinde anlamlı bir azalma görüldü (P<0.05). Tüm 1. yıl optik kalite değerlerinde artış gözlemlenmesine rağmen, sadece MTF 5, MTF 10 ve PSF değerlerinde istatistiksel olarak anlamlı artış izlendi (P<0.05). Gereç ve Yöntemler: Haziran 2021-Aralık 2022 tarihleri aras ında ingui- nal herni onarımı yapılan 260 hasta çalışmaya alındı. Preoperatif kronik ağrı ve postoperatif kronik ağrı görsel analog skala skoru değerlendirildi ve has- taların operasyon sonrası 3 ve 6. aylardaki sonuçlar ı kaydedildi. Ameliyat öncesi depresyon ve anksiyete değerlendirmeleri Beck Depresyon Envanteri ve Beck Anksiyete Envanteri kullanılarak yapıldı. Ameliyat öncesi, ameli- yat sırası ve ameliyat sonrası detaylar kaydedildi ve kronik postoperatif ağrı ile ilişkisi istatistiksel olarak analiz edildi. Bulgular: Postoperatif 3. ayda hastaların %22,3’ünde, 6. ayda ise hastalar ın %12,3’ünde ağrı devam etti. Hastaların %56,53’ünde preoperatif a ğrı mevcuttu ve bu hastalar ın %39,4’ünde kronik ağrı gelişti. Preoperatif kronik ağrı, postoperatif analje- zik kullanımı, ağrı süresi, kadın cinsiyet, reoperasyon, postoperatif kompli- kasyon ve postoperatif kronik a ğrı arasında istatistiksel olarak anlaml ı bir ilişki bulundu (p<0,001). Rekürren operasyon geçiren 24 hastan ın 21’inde postoperatif kronik a ğrı gelişti. Hastaların %53,4’ünde a ğrı nöropatik ka- rakterdeydi. Ameliyat öncesi anksiyete/depresyon ile kronik a ğrı arasında anlamlı bir ilişki gözlendi (p<0,001). Sonuç: Preoperatif ağrı, postoperatif akut ağrının şiddeti ve reoperasyon preoperatif dönemde sorgulanmal ı ve postoperatif kronik ağrıyı önlemek için yeterli ağrı palyasyonu sağlanmalı- dır. Additionally, it proposes an application process based on motor functionality for occupational therapists. A total of 288 children with CP were recruited for the study and administered the CVI Motor Questionnaire (CVI-MQ). The children's daily visual performance was assessed using the CVI Inventory and the Functional Vision Questionnaire (FVQ) considering the children's ambulatory status. The FVQ and the two factors extracted from the CVI Inventory significantly predicted visual functioning. Specific questionnaires related to CVI, applied with gross motor function in mind, can be valuable tools for occupational therapists to assess daily visual performance.Article Optical Coherence Tomography Findings in Primary Headache Disorders: Is Pain Duration a Clinical Correlate?(Taylor & Francis Ltd, 2024) Taşkıran Sağ, Aslıhan; Yazgı, Hare; Özülken, Kemal; Eroğlu, ErdalObjectiveGanglion cell layer thickness (GCLT) may be used as a potential marker for central neural changes. We compared GCLT by using spectral domain optical coherence tomography (SD-OCT) in patients with primary headache disorders and healthy controls. We seek whether there was any difference between the headache groups and whether any clinical parameters correlated to GCLT.MethodsFifty-three primary headache patients, 11 age and sex-matched healthy subjects were included in this cross-sectional study after power analysis. All subjects underwent SD-OCT. The duration of disorder, headache frequency, severity, duration of pain, presence of ocular pain, and accompanying symptoms have been collected.ResultsMean GCLT of the headache group was 15.7 +/- 3.8 mu m (mean +/- standard deviation), and the control group was 17.5 +/- 2.4. The difference was not statistically significant. When we compared the controls, migraine and tension-type headache patients' GCLT values, we found a significant difference (ANOVA, p = 0.001). Migraine patients had thinner GCLT compared to all non-migraine headache patients (p = 0.01). Intraocular pressure values of migraine patients and non-migraine patients were not statistically significantly different (p = 0.13). The only clinical parameter that correlated with GCLT was pain duration (r = -0.43 and p = 0.01). The patients with white matter lesions had thinner GCLT (p = 0.046).ConclusionOur results suggest that not long-term suffering from pain but migraine pathophysiology itself seems to affect neuroretinal tissue. Methods: A total of 28 patients diagnosed with migraine (24 female and 4 male) and 28 age matched-healthy controls (21 female and 7 male) were included in this cross-sectional study. Enhanced depth imaging optical coherence tomography scans of the macula and peripapillary regions were analyzed. Images were binarized using ImageJ software (National Institutes of Health, Bethesda, MD, USA) (Figure 1). The choroidal thickness (CT), total choroidal area (TCA), luminal area (LA), stromal area (SA), and the choroidal vascularity index (CVI) were compared between the groups. Results: The subfoveal and 750 microns nasal (N750) and temporal (T750) to the fovea CT were increased in migraine patients (P=0.004, P<0.001 and P<0.001, respectively). The mean subfoveal CVI value was 65.7 ± 6.8 in migraine patients and 63.6 ± 4.8 in healthy controls (P=0.159). The mean peripapillary CT at the temporal, nasal, and inferior regions were increased in migraine group (P=0.010, P<0.001 and P<0.001, respectively). The mean CVI values of temporal and nasal quadrant in the peripapillary region were significantly increased in the migraine group as compared with the control group (P=0.005 and P<0.001, respectively). The increase TCA and LA in both fovea and peripapillary regions were statistically significant in migraine group compared to healthy controls (P<0.05 for all). Both groups had similar measured values for other parameters (P >0.05 for all). Conclusion: According to our findings, increased CT, TCA, LA and CVI values may indicate a rebound vasodilatation in the ocular blood flow, as in the vasogenic theory contributing to pain.Article Clinical Behavior of Appendiceal Mucinous Neoplasia: 9 Years of Experience(2022) Demirbaş, Sezai; Ozer, M. Tahir; Dogan, Semra; Tulunay, Gökhan; Kaymak, SahinAppendiceal mucocele is known a mucinous neoplasia of the appendix. It’s etiology is not clear. For last 30 years it’s incidence has increased to 2,8 cases/million person from 0,6 cases per million. Even if different classification has been made over the years, WHO and AJCC classifications are frequently used. Objective: The aim of this study was to find the pathologically performing the discrimination between LAMN and HAMN in the extracted specimen and how much the scattering mucin influenced the surgery and patients’ follow-up. Material and Methods: Patients and Centers: In two different hospitals between 2012-2020, the patients admitted to the emergency department and diagnosed as acute abdomen were evaluated retrospectively. All patients were accepted in the emergency clinic and operated then followed in the general surgical clinic. The appendiceal mucocele diagnosis was based on perioperative observation of mucinous distention or mucin dissemination. After obtaining specimens, the type of the mucinous neoplasm of appendix diagnosis and assessment was made by histopathological investigation. Results: Appendix mucocele was detected in 19 of 2974 patients included in the study. Two additional patients had advanced pseudomyxoma peritonei (PMP) after previous appendectomy. 11 of the patients (52.4%) were female. The mean age was 63.8 years. The appendix was evaluated preoperatively as enlarged in fourteen patients. In 1 patient, this condition was belonging to the ovary. Adeno cancer in one patient and PMP in 2 patients were clinically detected. Histopathologically, the appendix size was 37.1x71.9 mm. Sixteen of the patients were reported as LAMN, 2 as HAMN, 1 as adenocarcinoma and 2 as PMP. The leukocyte, carcinoembryonic antigen (CEA) and CA 19-9 levels of the patients were found to be significantly higher than normal. The patients were followed up for an average of 30.2 months. Early postoperative complications were seen in 5 patients. Complications evaluated late were seen in 8 patients. Recurrence was detected in one of the patients during the follow-up. The average survival rate was 36.7 months, although it was slightly higher in women. While the effect of leukocytosis, CEA, CA 19-9 on mortality was not significant, but tumor size was (p<0.05). This study has a few limitations. These are the small number of patients, the retrospective evaluation of the patients, and the relatively short follow-up time. Conclusion: In this study, we found mucocele more frequently than seen in the literature. Histopathologically, low grade mucinous neoplasia (LAMN) was often encountered. Mucocele with acellular mucin scattering were also seen in approximately 20% of the patients. After simple appendectomy, no recurrence was observed during the follow-up. The leukocyte count, tumor markers, and tumor size were evaluated in terms of their effects on postoperative morbidity and mortality. The tumor size had a negative effect on survival only. Finally, the simple appendectomy operation was considered suitable for the treatment.
