dc.description.abstractPosterior pedikül vida enstrümantasyonu (PPVE) lomber omurga cerrahisinde iyi bilinen bir yöntemdir. Gerilme konsantrasyon seviyeleri, komşu segmentlere etkileri ve fizyolojik hareket ile ilişkisi gibi birçok belirsiz yönü olan alışılmış bel ağrısı probleminde yaygın olarak kullanılan bir tedavi yöntemidir. Ayrıca, PPVE'nin lomber omurganın biyomekaniğini nasıl etkilediğini anlamak önemli bir olgudur. Bu söz konusu anlayış için, faset eklemlerinin kısmen çıkarılmasıyla gerçek cerrahi prosedürü taklit eden L4-L5 segment seviyesinde PPVE olan lomber omurganın sonlu elemanlar (SE) modeli geliştirilmiştir. Faset eklemlerinin kısmen çıkarıldığı SE modelleri literatürde nadir bulunmaktadır. Bu SE çalışmasının amaçlarından biri, posterior enstrümantasyon gerilmelerinde ve faset eklem kuvvetlerinde hangi fizyolojik hareketlerin en fazla artışa sahip olduğunu belirleyerek PPVE sisteminin lomber omurganın füzyon öncesi/sonrası biyomekaniği üzerindeki etkisini araştırmaktı. Daha sonra PPVE'nin özellikle ekstansiyon ve lateral eğilme hareketinde L3-L4 komşu seviyesindeki faset eklem kuvvetlerinde sırasıyla %35 ve %23'ün üzerinde artışa neden olduğu sonucu elde edilmiştir. Komşu seviyedeki bu faset eklem kuvvetleri artışı, posterior lomber interbody füzyonundan sonra komşu segment dejenerasyonunun (KSD) meydana geldiği veya hızlandığı hipotezi için bir ipucu sağlayabilir. Ayrıca, bu çalışmada posterior enstrümantasyon üzerindeki tepe Von Mises gerilmelerinin analizleri, maksimum posterior enstrümantasyon gerilmelerinin sırasıyla 272.1 MPa ve 263,7 MPa olarak füzyondan önce lateral eğilme ve fleksiyon hareketinde meydana geldiğini göstermiştir. Öte yandan PPVE operasyonu sırasında cerrahlar kendi deneyimlerini kullanarak hastanın omurga eğriliğine göre çubukları bükerler. Dolayısıyla bu bükme nedeniyle çubuklar üzerinde artık gerilmeler oluşmaktadır. PPVE üzerine birçok SE tabanlı biyomekanik araştırma yapılmış olmasına rağmen, çubuk konturlamasından kaynaklanan artık gerilmelerin yapı gerilmeleri üzerindeki etkilerini değerlendiren çok az araştırma vardır. Bu nedenle, bu çalışma aynı zamanda bir French bükücü ve in-situ bükücü kullanılarak konturlanmış çubuklar içeren PPVE üzerindeki artık gerilmenin etkilerini araştırmayı ve farklı fizyolojik hareketler için düz ve konturlu çubukların gerilme artışındaki farklılıkları karşılaştırmayı amaçlamıştır. Bu bağlamda, PPVE için L4-L5 lomber omurga segmentinin bir SE modeli geliştirilmiş ve artık gerilmelerin çubuklar ve yapı gerilmeleri üzerindeki etkileri farklı bükme yöntemleri kullanılarak araştırılmıştır. Simülasyonlarda, bir French bükücü tarafından şekillendirilen artık gerilimli çubukların, bir in-situ bükücü tarafından şekillendirilenlere göre PPVE'de önemli ölçüde daha fazla gerilme artışı ile sonuçlandığı gözlemlenmiştir. Çalışmanın sonuçları, farklı fizyolojik hareketler için artık gerilmeler nedeniyle PPVE'deki gerilme artışının PPVE kırılma riskini artıracağını vurgulamıştır. Mevcut çalışmada verilen SE modelleme yaklaşımı, PPVE'nin yorulma ömrü ve kırılma çalışmaları gibi daha sonraki araştırmalar için temel bir araç olarak kullanılabilir. Bu modeller, belirsiz yönleri ortadan kaldırarak PPVE'nin etkilerinin daha iyi anlaşılmasını sağlayarak gelecekteki farklı çalışmalar için de kullanılabilir.en_US
dc.description.abstractPosterior pedicle screw instrumentation (PPSI) is well-known method in lumbar spine surgery. It is a common treatment method for widespread low back pain problem, which has many uncertain aspects such as stress concentration levels, effects to adjacent segments and relationship with physiological motion. In addition, understanding how PPSI affects biomechanics of lumbar spine is an significant phenomenon. For this understanding, a finite elements (SE) model of lumbar spine with PPSI at L4-L5 segment level was developed by partially removing facet joints which imitates actual surgical procedure. SE models with partial removal of facet joints are rare in the literature. One of the aims of this SE study was to investigate the influence of PPSI system on the biomechanics of the lumbar spine before/after fusion by determining which physiological motions have the most increase in the posterior instrumentation stresses and facet joint forces. Afterwards, it was concluded that PPVE caused an increase of over 35% and 23%, respectively, in facet joint forces at the L3-L4 adjacent level, especially in extension and lateral bending motion. This facet joint forces increment at the adjacent level could provide a clue for hypothesis of that adjacent segment disease (ASD) was occurred or accelerated after posterior lumbar interbody fusion. Furthermore, analyses of peak Von Mises stresses on posterior instrumentation in this study showed that the maximum posterior instrumentation stresses 272.1 MPa and 263.7 MPa occurred in lateral bending and flexion motion before fusion, respectively. On the other hand, during PPSI operation, surgeons bend rods according to spine curvature of a patient by using their own experiences. Therefore, residual stresses developed on the rods due to this bending. Despite the fact that many FE-based biomechanical research have been conducted on PPSI, there are few investigations assessing the effects of residual stresses arising from rod contouring on construction stresses. Thus, this study also aimed to investigate effects of the residual stress on PPSI that use rods contoured by using a French bender and an in-situ bender, and to compare differences in stress increment of straight and contoured rods for different physiological motions. In this sense, a FE model of L4-L5 lumbar spine segment was developed for PPSI and effects of the residual stresses on rods and construct stresses were investigated by using different bending methods. In the simulations, it was observed that rods contoured by a French bender with residual stress resulted in significantly more stress increment in PPSI according to ones contoured by an in-situ bender. The results of the study emphasized that stress increment in PPSI due to the residual stresses for different physiological motions would increase risk of PPSI failures. FE modelling approach given in the current study could be used as a fundamental tool for the further investigations such as fatigue life and failure studies of PPSI. This models could be used also for different future studies by providing better understanding of the effects of PPSI by clearing up the uncertain aspects.en_US
